It seems both block and chunk are out of range.
How many blocks are there in your partition?
And what's your dev->nChunksPerBlock?

You'd better turn on YAFFS_TRACE_SCAN and YAFFS_TRACE_SCAN_DEBUG to
genertate more logs while mount.
i.e. echo +scan +scan_debug > /proc/yaffs
